1. Please ensure that you have the following
supporting documentation ready as these
must be uploaded with your online
application:
•Copy of learner's birth certificate/ID
or passport
• Copy of learner's latest progress report
• Copy of parent's/legal guardians' ID
or passport

2. You will receive email communication
acknowledging receipt of the application
and will be notified if the learner
has been accepted or declined.

3. Successful applicants will
need to do the following:
• Sign and submit the
additional documentation
• Pay the enrolment fee

Learners medical consent

The lIn a critical medical situation, please bear in mind that there may not be time to refer to the learners records. The online school therefore reserves the right to utilise the quickest medical service available.

I/We, being the parent/legal guardian/s of all learners in this application, hereby agree that a medical practitioner may provide emergency treatment as and when required.

Accountholder declaration

I/We, the accountholder/s, hereby certify that the information provided in this application for admission is complete and accurate. I/We acknowledge that enrolment is subject to, inter alia, signing a learner admission contract that contains the detailed terms, conditions and requirements for admission.

I/We hereby authorise the online school and/or any of its associates to conduct any credit enquiries as may be necessary from time to time.

I/We ackowledge that I/we have read the online school-specific policies and rules and will accept an offer of placement for our child/ren at the online school in accordance with the terms and conditions as set out therein.

If your application is successful, the following documents will need to be signed per successful applicant.
